Description of Services:
  • Offers timely access to family and community primary health care
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
  • Administers various health programs including smoking cessation and a memory clinic
  • Coordinates regular visits from the Screen for Life Coach for breast screening mammograms
Offers primary health care including but not limited to:
  • Chronic disease education and prevention
  • Disease management
  • Health promotion
  • Mental health assessment

Fees: Most services covered by OHIP
Eligibility - Population(s) Served:
  • Enrolled patients of the JE Stokes Medical Centre and the Aguasabon Medical Clinic
  • Must have a valid Ontario Health Insurance Plan (OHIP) card or other provincial health coverage
Application:
  • Requires booked appointment
  • No referral required for most services
  • New patients may roster with a Physician by completing enrollment form available at office
