Description of Services:	Provides education and services for the prevention, management and treatment of diabetes Provides case management and screening services Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes Advises on developing and implementing an approach to care and management of diabetes Communicates patients' concerns to family physician Assists individuals in finding appropriate services Provides referrals to other health care professionals

Fees:	Most services covered by OHIP

Eligibility - Population(s) Served:	Patients of the JE Stokes Medical Centre and the Aguasabon Medical Clinic living with diabetes or at-risk of developing diabetes Must have a valid Ontario Health Insurance Plan (OHIP) card or other provincial health coverage

Application:	Requires booked appointment No referral required for most services New patients may roster with a Physician by completing enrollment form available at office
