Description of Services:	Indigenous Care Coordinators Provides a range of inpatient services to Indigenous Peoples accessing health and mental health services through the Emergency Department and who are admitted into the Hospital, including: Liaising between TBRHSC and local community organizations Connecting to translation and interpretation services in Ojibwe, Cree, and Oji-Cree Supporting inpatients during their stay at TBRHSC Liaising and advocating for patients and their families with their health care team Providing information and education in a culturally sensitive way Providing a connection with Non-Insured Health Benefits, access to iPad and book lending libraries, link patients to traditional care practitioners, and act as liaisons with the Compliments and Concerns process Supporting discharge planning with physician/health care practitioner through an interdisciplinary team approach Linking patients and families to helpful community resources, processing referrals, and providing clear and accurate information on available community-based services to support patients upon discharge Working closely with Indigenous Patient Navigators and Dilico Discharge Planners Indigenous Patient Navigators Provides services to Indigenous patients in out-patient areas such as the Renal Unit, Regional Cancer Care Unit, and Centre for Complex Diabetes Care and other areas as requested, including: Interpretive services in Cree, Ojibway, and Oji-Cree anywhere in the Health Sciences Centre Support before, during, and after clinical appointments Linking patients and families to community resources Tele-visitation services with remote family members Information and education in a culturally-sensitive manner Liaison and advocate between the care team, patients, and families Assistance with discharge planning Indigenous Patient Navigator, Renal Services Provides interpretation, advocacy, and support for Indigenous renal (dialysis, kidney disease) patients during renal clinic visits Indigenous Patient Navigator, Regional Cancer Care Provides translation/interpretation, advocacy, and support for Indigenous cancer patients Indigenous Liaison Worker, Centre for Complex Diabetes Care (CCDC) Provides translation/interpretation, advocacy, and support for Indigenous patients at the CCDC Traditional Wellness Lead Supports Indigenous patients, families, and staff by ensuring access to traditional and spiritual care within the hospital Collaborates with Elders, Knowledge Keepers, and partners, to help create a culturally safe environment that supports the whole person Bridges traditional knowledge with modern medicine, ensuring Indigenous patients and families feel respected, supported, and safe in their healthcare journey Provides cultural safety education and training Organizes cultural events and ceremonies Supports patients and families during sensitive times

Eligibility - Population(s) Served:	Patients requiring the services of an Indigenous Care Coordinator, Indigenous Patient Navigator, Indigenous Liaison Worker or Traditional Wellness Lead while receiving care at the hospital
