Description of Services:	The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues Services and Programs: Chronic Pain Self-Management Program Diabetes Education Classes Dryden Diabetes Centre Falls Prevention Exercise Program Heart Health Program Memory Workshop Mental Health Counselling Nutritional Counselling Patient Navigator Primary Health Care
