Frais:	Service - there may be fees associated with the prosthesis; staff can provide more information at the appointment

Admissibilité / population desservie:	Individuals 15 years of age and older residing in Northwestern Ontario and Requiring assessment for suitability for a prosthesis or Wanting information about a prosthesis and prosthetic training
