Description of Services: Rehabilitation team that provides support for individuals who require a prosthesis
  • Provides weekly clinics by a Physician, Physiotherapist, Occupational Therapist, Prosthetist, and Social Worker
  • Offers an Amputee Clinic Monday afternoons via Ontario Telemedicine Network as required
  • Provides assessment for suitability for a prosthesis and/or modifications of an existing prosthesis
Requirements
Fees: Service - there may be fees associated with the prosthesis; staff can provide more information at the appointment
Eligibility - Population(s) Served:
  • Individuals 15 years of age and older residing in Northwestern Ontario and

  • Requiring assessment for suitability for a prosthesis or

  • Wanting information about a prosthesis and prosthetic training
