Program: Atikokan Family Health Team - Zuke Rd - Smoking Cessation Program

Description of Services:
  • Offers a smoking cessation program to assist individuals in quitting tobacco use
  • Conducts initial assessment to determine the patient's smoking habits and health profile to provide the appropriate treatment
  • Provides self-management support and strategies
  • Assists patients released from hospital to remain smoke-free after discharge
    • Offers the following support and services:
    • Education and information
    • Counselling
    • Nicotine Replacement Therapy (NRT) that can include a patch, lozenges, gum and inhalers
    • Prescriptions for Champix or Zyban (through FHT Pharmacist)

    Note: Patients will qualify for up to six months of free nicotine replacement therapy
