Description of Services:
  • Offers a 6-week workshop for individuals living with chronic or long-term health conditions such as diabetes, arthritis, stroke, cancer, colitis, lung disease, fibromyalgia, Parkinson's, heart disease and other conditions
  • Educates individuals on how to manage their symptoms to become more confident and improve their quality of life
  • Each participant receives a free copy of the "Living a Healthy Life with Chronic Conditions" Wellness Guide
  • Facilitated by Registered Nurse and community members living with chronic disease
