Description of Services:
  • Offers holistic practical services for individuals with mental health/addictions issues accessing the Morningstar Centre or Emergency Department to aid in their healing path
  • Focuses on improving the overall health of individuals and reducing the number of repeat visits to the emergency department
    • Services provided include but are not limited to:
    • Accompaniment to medical appointments
    • Assisting clients to obtain identification (ID)
    • Assistance with updating Ontario Hospitalization (OHIP) cards
    • Completion of housing applications

    Eligibility - Population(s) Served:
    • Individuals with mental health/addictions issues

    • Individuals accessing the Emergency Department at Lake of the Woods Hospital or the Morningstar Centre and requiring aftercare support
    Application:
    • Referral can come from Emergency Department or Morningstar Centre staff

    • Intake assessment and completion of Inter-agency Intake required
