Description of Services:
  • Manages 181 market value and rent geared to income apartment units for seniors or younger individuals with a disability who are capable of living independently with or without support
    • Provides the following support services:
    • Assistance of a Case Manager
    • Emergency response
    • Housecleaning and laundry services
    • Life enrichment
    • Meals
    • Personal care
    • Medication reminders
    • One-to-one support
    Requirements
    Fees: Service; Fee assistance available for 70% of Support Services; assistance is provided by Ministry of Health and Long Term Care
    Eligibility - Population(s) Served: Individuals 60 years of age and older

    • Must be a Canadian citizen or landed immigrant

    • Must be a resident of Ontario for at least a year

    • Individuals under 60 years of age must have a disability and be able to live independently with or without support

    • Applications can be picked up at St Joseph's Heritage front desk
