  • Helps individuals to participate in meaningful life roles at, or as close as possible to, the level they did prior to their acquired brain injury
  • Provides flexible services to meet the diverse needs of individuals who require rehabilitation and support services
  • Provides referrals to the residential services available in Thunder Bay
  • Provides Community Rehabilitation and Outreach Services in Thunder Bay, Kenora, Fort Frances and Dryden
  • Offers Assisted Living Services in the following locations:
    • Castlegreen Site
    • Cumberland Place
    • McKellar Place
  • Requirements
    Fees: None
    Eligibility - Population(s) Served: Individuals 16 years of age and older who have an acquired brain injury
    Application:
    • Referrals by family, health professional, physician, lawyer, or self-referral

    • Intake process includes completed and signed "Application for Service" (AFS) form which can be found on the website

    • AFS form can be signed by substitute decision maker
