Description des services:	Offers various workshops for individuals living with chronic or long-term health conditions such as diabetes, arthritis, stroke, cancer, colitis, lung disease, fibromyalgia, Parkinson's, heart disease and other conditions Educates individuals on how to manage their symptoms to become more confident and improve their quality of life Offers workshops online Programs include: Chronic Disease Self-Management Program (6-week program) Diabetes Self-Management Program (6-week program) Chronic Pain Self-Management Program (6-week program) Getting the Most from Your Healthcare Appointment (1.5 hrs) ABC Health Matters workshops (90 mins each): Organizing Care Managing Stress with a Rare or Chronic Disease Healthy Caregiving

Admissibilité / population desservie:	Individuals 18 years of age and older with chronic or long-term health conditions and their caregivers Participants must be able to set goals and function in a group setting; for example, the program would not be appropriate for someone with significant dementia or memory impairment, or for someone who would be disruptive in a group
