Program: Sandy Lake Health Authority - Diabetes Prevention Program
Organization: Sandy Lake First Nation
Description of Services: Provides education and services for the prevention, management and treatment of diabetes
  • Works in partnership between the community and diabetes/health researchers to address the challenges of diabetes
  • Collects diabetes and related data from community members through a diabetes research team twice annually
School-Based Diabetes Prevention Program (SBDPP)
  • Provides a culturally-sensitive diabetes prevention program to students in grades 3 and 4
  • Offers a curriculum consisting of information on diabetes, healthy eating and daily physical activities
Community-Wide Diabetes Prevention Activities
  • Provides education and information to community members through health fairs, music festivals, sports tournaments, nutrition awareness weeks and other special events
  • Offers a morning snack program for all students at both the elementary and high school
  • Collaborates with the Northern Store to provide shelf labels on healthy food choices
  • Holds 2 weekly radio broadcasts, one geared to youth, to educate and raise awareness about diabetes and other health issues
  • Maintains walking trails around the community
  • Sponsors a walking club and various special events such as the poker walk
Also offers the following services:
  • Provides case management and screening services
  • Offers basic foot care
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals
