Description of Services: Provides a multi-disciplinary team including representatives from social work, pharmacy, nursing, dietitian, family medicine, and the Alzheimer's Society to address memory issues
Services provided include:
  • Assessment of issues such as delirium and depression based on a clinical reasoning model
  • Assessment of functional and cognitive impairment
  • Considerations in individual's fitness to drive
Interventions may include but are not limited to:
  • Medication changes
  • Referrals to programs and support
  • Driving status considerations

Application: Referral required and must have a primary care provider
