Description of Services:	Provides a 36-unit seniors supportive housing facility Consists of studio units, single-bedroom and two-bedroom suites for independent or supported living Includes a large reception area and fireplace lounge, common dining hall, recreation area and laundry room Minimal to moderate care services include: Housekeeping and personal care assistance from Personal Support Workers Meal supports in-home or congregate dining Social activities and events

Eligibility - Population(s) Served:	Seniors 60 years of age and older Must be able to live independently within an apartment and require intermittent partial support to perform activities of daily living Physician's form is required in order to access support services
