Description of Services: A specialized outpatient program to support individuals with wounds that are slow to heal
  • Supports the physical, mental, emotional, environmental, nutritional and financial needs of the individual
  • Provides consultation with other healthcare providers as needed
  • Provides Telemedicine appointments as appropriate
  • Offers a multidisciplinary team consisting of Nurse specialized in wound, ostomy and continence, Physiotherapist, Occupational Therapist, Dietitian, Social Worker, Chiropodist, and Physician

Application: Referrals can be made by the client, family, caregiver or healthcare professionals
