Admissibilité / population desservie:	Individuals 65 years of age and older who have been discharged from the Lake of the Woods District Hospital and are taking 4 or more medications or at least 1 Institute for Safe Medication Practices (ISMP) medication

Procédé et formulaires:	If deemed necessary, arrangements made upon referral from Lake of the Woods District Hospital
