Description of Services: Social, recreational, and therapeutic activities for people living with a chronic or disability in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

Additional services include:
  • hot meals and snacks
  • medication reminders and assistance
  • assists with activities of daily living
Requirements
Fees: $12 per day includes meal and snacks * subsidy available for eligible participants * additional fee for transportation
