St Joseph's Care Group - St Joseph's Health Centre North - Mental Health Outpatient Program
Description of Services:
  • Provides safe and affordable housing in the community for individuals with a serious mental illness and who require treatment, rehabilitation, and support
  • Works in partnership with three non-profit landlords, Northern Linkage Community Housing Incorporated, Lutheran Community Housing, and Matrix Affordable Housing
  • Provides high and medium levels of supportive accommodation

  • Community Homes for Opportunity (CHO):
    • Provides accommodations in private homes for individuals with serious mental illness who require long-term care and community based rehabilitation
    • Partners with six Home Operators licensed by the Ministry of Health and Long-Term Care who can accommodate up to 41 clients
    • Residents receive treatment and rehabilitation services from a team of health care workers

    Eligibility - Population(s) Served: Primary diagnosis of serious mental illness
    Application: Referrals are required and may be from hospitals, agencies, or within St Joseph's Care Group
